Adult Learner Employment Verification

In order to receive the Adult Learner Scholarship we will need a letter from church each year you receive the award stating that you are employed at the church full-time, and as such, can only attend school part-time.

Church Matching Form

In order for ACU to match funds given by your church we must receive a completed Church Matching Scholarship application.

Citizenship Status- Secondary Confirm

The United States Citizenship and Immigration Services (USCIS) of the Department of Homeland Security has not yet verified that you are an citizen or eligible noncitizen. You may need to provide proof of eligibility to the financial aid office by submitting a copy of your Passport, birth certificate, or certificate of naturalization. Please contact the financial aid office at [email protected] for questions.

Default Resolution Letter
The federal government has notified us that your student loans are in default. Students who are in default are not eligible for federal financial aid. In order to receive financial aid you must contact your lender to make payment arrangements, obtain a letter stating the student loans are now in good standing and submit this letter to the financial aid office.

Enrollment History Review

The Department of Education has flagged your FAFSA for further review based on unusual prior enrollment history. You are required to submit a copy of yours transcripts to the financial aid office for any and all schools during the review period. For 2015-2016 FAFSA’s the review period will be all schools attended during the 2012-2013, 2013-2014, and 2014-2015 academic years.

Entrance Counseling

In order to receive federal student loans, students must complete entrance counseling for each school they attend. Our records indicate you have not yet completed entrance counseling for ACU. You can complete entrance counseling at studentloans.gov.

FAFSA

In order to be considered for institutional funds and any federal or state funds you must complete the Free Application for Federal Student Aid at www.FAFSA.ed.gov. Our records indicate that we have not received your FAFSA for the 15/16 academic year. Be sure to add our school code to your FAFSA so that your information comes to ACU. If you do not plan to complete a FAFSA for this year, please complete a 15/16 FAFSA waiver and submit it to the financial aid office.

Missing Household Information

The FAFSA has indicated that you did not complete the number of persons in your household. Please return to your FAFSA and make the proper corrections to your information.

Missing Parent Signature

The FAFSA has been completed, but is missing your parent’s signature. Please return to the FAFSA and have your parent sign the FAFSA with their PIN, then resubmit the FAFSA.

Missing Student Signature

The FAFSA has been completed, but needs your (student) signature. Please return to the FAFSA and sign it with your PIN, then resubmit the FAFSA.

Music Scholarship Agreement

Congratulations on your award. In order for your music scholarship to disburse to your account you must sign your scholarship agreement. Please return your signed agreement to the financial aid office.

NSLDS Loan Discharge

Your FAFSA indicates that one or more of your student loans have been discharged. Please contact the financial aid office to discuss your financial aid eligibility.

Parent Date of Birth Mismatch

The date of birth provided on the FAFSA for your parents does not match the date of birth in the Social Security Administration’s records. Please correct the date of birth or social security number provided. If the information on the FAFSA is correct please reaffirm the information and submit the FAFSA again. If the information is correct you will also want to contact the Social Security Administration to make sure they correct it in their records. You may contact them by visiting www.socialsecurity.gov.

Parent Income Incomplete

Your parent’s income section on the FAFSA is incomplete. Please return to the FASFA and enter the missing information under the “Financial” section of the FAFSA, then resubmit the FAFSA. Please note, you can use the IRS Upload option to link the income information from the IRS to FAFSA.

Parent IRS Upload

Your FAFSA was selected for further review. To verify your parent’s income, please go back to the FAFSA‘s financial information section, and click the box to use the IRS Upload. Follow the directions to link your parent’s tax information from the IRS site to FAFSA, then resubmit the FAFSA.

Parent Marital Status Review

Please review the marital status of your parents provided in Item 58 on the FAFSA. If your parents are not married, provide only the income for the parent who supports you. If your parents are married please correct the marital status question.

Parent SSN Mismatch

FAFSA is unable to verify your parent’s social security number provided. Please confirm the social security number is correct and make any necessary corrections. If you believe the SSN provided is correct, your parent must contact the Social Security Administration. They may do so by visiting www.socialsecurity.gov.

Parent Tax Filing Status

Your FAFSA indicates that your parents will not file a tax return, however the income reported for your parent’s income appears to be over the minimum amount required to file a tax return. Please review your FAFSA and make any necessary corrections.

Parent Taxes Above Threshold

Your FAFSA indicates there may be an error in the amount reported by your parents as taxes paid. Please correct or confirm the amount of your adjusted gross income and taxes paid on your FAFSA and resubmit the corrections.

Parent's Social Security Card

FAFSA is unable to verify your parent’s social security number. Please provide a copy of the social security card to the financial aid office.

Parent's Tax Transcript

Your FAFSA was selected for further review. To verify your parent’s income, a copy of the Tax Return Transcript is needed. Please go to irs.gov to order a copy. A copy of this transcript must be provided to the financial aid office.

Pell Lifetime Eligibility Review

The FAFSA has indicated that you may have reached the limit of the amount of Pell Grant you can receive. The financial aid office will review your eligibility and make any adjustments needed. No action is required by the student.

Plus Master Promissory Note

If your parent is approved for a Parent Plus loan, they must complete the Master Promissory Note in order for the funds to disburse to the school. Parents may complete the Master Promissory Note by visiting studentloans.gov and following the instructions on the website.

Plus Loan Application

A Plus loan is a credit based loan that parents can borrow to help students fund their education. In order to receive a Plus Loan your parents must apply and be approved. Parents may apply by visiting studentloans.gov and following the instructions on the website. If a parent is denied, the student will become eligible for an additional Stafford loan.

PR Band Scholarship Agreement

Congratulations on receiving the PR Band Scholarship. Before these funds can disburse to your account, you must sign and return your scholarship agreement. Please return your signed agreement to the financial aid office.

President's Scholarship Agreement

Congratulations on receiving the President’s Scholarship. Before these funds can disburse to your account, you must sign and return your scholarship agreement. Please return your signed agreement to the financial aid office.

Proof of Selective Service Registration

The FAFSA has indicated that you have not registered for Selective Service. Please contact the financial aid office for further information.

SAP Appeal Form

You have not maintained Satisfactory Academic Progress and are no longer eligible for financial aid. You have a right to appeal this status. Please submit the SAP Appeal Form to the financial aid office in order to be considered for reinstatement of financial aid.

Scholarship Appeal Form

You have not maintained the GPA requirements for one or more of your scholarships and as a result your scholarship has been revoked. If extenuating circumstances prevented you from meeting the requirements of you award, you may be entitled to appeal. Please submit the Scholarship Appeal Form to the financial aid office in order to be considered for scholarship reinstatement.

Servant Leaders' Agreement

Congratulations on receiving the Servant Leaders Scholarship. Before these funds can disburse to your account, you must sign and return your scholarship agreement. Please return your signed agreement to the financial aid office.

Signed Athletic Scholarship Agreement

Scholarship Thank You Letter

You have been awarded a scholarship from one of ACU’s private donors. Please write a thank you note addressed to “Dear Donor,” and return the letter to the financial aid office. Suggested text for your letter could include your academic plans, how the scholarship will help you, and your involvement here at ACU.

Special Circumstance Form

In order for the financial aid office to process your Petition for Special Circumstances, we must receive your completed Special Circumstances form. Please submit the requested document to the financial aid office.

Student Birth Certificate

The FAFSA was unable to verify your citizenship. Please provide a copy of this document to the financial aid office.

Student Certificate of Naturalization

The FAFSA was unable to verify your citizenship. Please provide a copy of this document to the financial aid office.

Student Citizenship Confirmation

The FAFSA could not confirm your citizenship status. In order to resolve this you must submit proof of your citizenship status to the financial aid office such as a certificate of naturalization, passport, or birth certificate.

Student Date of Birth Missing

The date of birth you provided on the FAFSA does not match the date of birth in the Social Security Administration’s records. Please correct the date of birth or social security number provided. If the information on the FAFSA is correct please reaffirm the information and submit the FAFSA again. In order to receive the Federal TEACH Grant, you must complete TEACH Grant Counseling each year. We have not received your 2015-2016 TEACH Grant Counseling. You can complete this agreement online at teach-ats.ed.gov.

Tuition Waiver Form

Our records indicate that you are an employee, spouse of an employee, or an eligible dependent of an employee and may be eligible for a tuition waiver. Please contact Human Resources to complete the tuition waiver form.

U.S. Passport or Proof of Citizenship

The FAFSA could not verify your citizenship status. You must provide a copy of your US Passport of other proof of citizenship to the financial aid office.

VA Eligibility Letter

Before the financial aid office can certify you for your Veteran’s benefits, you must submit a copy of your certificate of eligibility. You may apply for benefits online using VONAPP at https://www.ebenefits.va.gov/. Once you apply you will be able to view your eligibility information online.

Verification of Dependency Status

You indicated on your FAFSA that you were either an orphan, a ward of the court, in foster care at the age of 13, an emancipated minor, or in legal guardianship. You are required to verify this status. Please complete the Dependency Status Confirmation Form and return to the financial aid office with appropriate documentation.
